Description:
Adult Day Health - Whatcom Region. Day shift, & 0830-1600 & F & 7 (.0875) hours per week. & Open 3/16/2006. $13.93 per hour minimum. & $17.42 per hour midpoint. & $20.90 per hour maximum. THIS POSITION IS OPEN TO ALL CURRENT SJH EMPLOYEES ONLY. Demonstrates compassion, respect and caring while working within the scope of ADH Nurse. Works collaboratively with other healthcare team members toward the best outcomes for the client and the organization. Functions as clinician, caseworker and consultant on the interdisciplinary care team. Helps to plan, implement and evaluate the care for vulnerable adults to ensure the multi-disciplinary treatment plan meets state and hospital standards. Exercises creative stewardship of resources. Contributes toward realization of the Adult Day Health vision and PeaceHealth mission and values. Familiarity with nurse practice act and Washington Administrative Code relevant to adult day services. Able, under RN supervision, to assist in the collection of assessment data, creation of care plans and evaluation of outcomes. Desktop computing experience preferred. Degree or diploma from accredited school of practical nursing. WA State LPN license. CPR certification within probationary period. Washington State Food Handler’s permit. Continuing education or certification in Geriatric Nursing preferred. Experience working with older adults and/or adults with disabilities preferred, particularly in a community health setting. Desktop computing experience preferred. Excellent organizational skills. Competency in point-of-care testing for blood glucose, urinalysis, etc. Must work well with staff and volunteers in an interdisciplinary setting. Flexibility and willingness to perform a broad range of services necessary. Ability to communicate clearly and respectfully with older adults, adults with disabilities, families, and other providers. Attentive and able to respond to many demands simultaneously. Responds to special needs and demands in a patient, compassionate and cheerful manner. Ability to perform physical requirements of the job.
